OUR MISSION

Our focus in the clinical practice would be to improve as a bedside clinician and increase formal education of young trainees so that India can foresee a better Rheumatology future.

There is lack of formal Rheumatology education in the medical training. This has led to unnecessary tests and treatment for these patients. There is marked anxiety and stress associated with the diagnosis of autoimmune diseases. Timely and appropriate diagnosis and counseling can help these patients to overcome this stress.

Joint Injections

Joint and Soft tissue injections

Joint injections in arthritis leads to a prompt relief from joint pain and swelling. It can prevent permanent damage to joints by controlling inflammation quickly. They are used when few joints (1 to 4) are involved or if one or two joints in a polyarticular disease are not responding to oral medications. There are many myths related to joint and soft tissue infections. They do not cause any damage to the joint except for a minor risk of infection. Giving the medicines directly into the joint results in lesser side effects compared to giving them in the form of oral tablets or injections. Many people believe that removing fluid from the joint or giving injections damages the joint. This is not true. Joint and soft tissue injections are harmless and generally result in better and rapid control of symptoms.

In complex cases of systemic autoimmune diseases sometimes we require tissue biopsies to confirm the diagnosis. It helps to establish the diagnosis most firmly for further management and is less likely to cause major complications.

Various tissue biopsies done are:

  • Kidney biopsy for SLE or vasculitis related kidney diseases
  • Sural Nerve biopsy for Vasculitic Neuropathy
  • Skin Biopsy for panniculitis, skin rashes as and when required
  • Minor Salivary Gland biopsy for Sjogren’s syndrome
  • Vastus lateralis Muscle Biopsy for Idiopathic Inflammatory Myositis
  • Bone Marrow Aspiration and Biopsy as and when required for the diagnosis

This is a rapidly evolving bedside assistant for a rheumatologist. It helps a clinician to refine his clinical skills with the help of images created by ultrasound. It acts as eyes to see what is exactly going on inside the tissues. It can be used to guide injections to the exact site of inflammation and prevent damage to surrounding structures.

At present we are using online software for data collection. We are looking forward to evolve in this segment in the coming years.

At SGPGI, our training institute, we have been trained in reporting and interpreting these tests. Being from an institute where laboratory immunology is an interesting skill to preserve and develop upon in the future.

Physiotherapy is a part and parcel of various musculoskeletal (MSK) diseases. Many patients with arthritis and other MSK related diseases need proper exercise training for muscle strengthening and for maintaining range of motion of joints. Collaborating it with rheumatology consultation can help the patient to overcome from these diseases with a better quality of life.
